Bed Bug or Roach? How to Tell the Difference

If you’ve spotted a small, dark insect scurrying across your mattress or hiding in a crack near your bed, you’re probably trying to figure out whether you’re dealing with bed bugs or cockroaches. The two pests look surprisingly similar in their early life stages, but they behave differently, leave different evidence behind, and require completely different treatment strategies. Knowing which one you have changes everything about how you respond.

How to Tell Them Apart by Appearance

Adult bed bugs and adult cockroaches are fairly easy to distinguish. Bed bugs are small, flat, and oval, roughly the size of an apple seed (about 5 to 7 millimeters). They’re reddish-brown and wingless. Cockroaches, even the smaller German cockroach species most common indoors, are larger, more elongated, and have visible wings (though German cockroaches rarely fly). Adults aren’t usually the source of confusion.

The real identification challenge comes with nymphs, the immature forms of both insects. German cockroach nymphs and young bed bugs are both tiny, pale or translucent, and tend to hide in dark crevices. Two features help you tell them apart quickly: body shape and antennae. Bed bug nymphs have a wider, rounder oval body, while cockroach nymphs are more elongated and narrow. The antennae are the giveaway. Bed bugs have short, stubby antennae that are easy to miss. German cockroach nymphs have long, thread-like antennae that are constantly moving and clearly visible even at small sizes.

Where Each Pest Hides

This is one of the most reliable ways to narrow down what you’re dealing with, because bed bugs and cockroaches have fundamentally different needs.

Bed bugs stay close to where people sleep. Their hiding spots are almost always within a few feet of a bed or couch: mattress seams, box spring crevices, headboard joints, nightstand drawers, and behind picture frames near the bed. They don’t need food or water beyond human blood, so they have no reason to be in your kitchen. If you’re finding insects tucked into your mattress seams or along the piping of your pillows, bed bugs are the more likely culprit.

Cockroaches need food, water, and warmth. Their permanent harborage is almost always the kitchen or bathroom, near sinks, drains, and food sources. You’ll find them behind refrigerators, under sinks, inside cabinet hinges, and around garbage areas. When an infestation grows large enough, cockroaches start showing up in unusual locations like inside smoke alarms, along ceiling lines, behind wall pictures, and inside electrical outlets, but they’ll keep returning to areas with reliable food and moisture. If the insect you found was in or near a kitchen or bathroom, cockroaches are far more likely.

Droppings and Other Evidence

Both pests leave droppings behind, but the droppings look and feel different. Bed bug fecal spots are black (not red, because the blood is already digested) and appear as small ink-like dots, often in clusters of ten or more near their hiding spots. They have a smooth texture because they’re made of dried liquid. German cockroach droppings, by contrast, feel granular and gritty because they contain solid waste. Cockroach droppings look like dark specks or smears, often resembling ground pepper, and you’ll typically find them along cabinet edges and drawer corners in the kitchen.

Bed bugs also leave behind molted skins as they grow through five nymph stages before reaching adulthood. These shed skins look like empty, translucent shells in the shape of the bug itself, and you’ll find them in various sizes depending on the life stage. Bed bug eggs are pearl-white, about the size of a pinhead, and develop visible dark eyespots after five days.

Cockroaches leave behind egg casings called oothecae, which are small, purse-shaped capsules that are brown or dark reddish-brown. A single German cockroach egg case holds around 30 to 40 eggs. Finding one of these in a kitchen cabinet is a strong indicator of a cockroach problem.

Bites and Skin Reactions

This distinction is straightforward: bed bugs bite, cockroaches generally don’t. If you’re waking up with new bites that weren’t there when you went to sleep, bed bugs are the primary suspect.

Bed bug bites cause redness, itching, and minor swelling. They often appear in a characteristic pattern that dermatologists call “breakfast, lunch, and dinner,” a line or cluster of three or more bites in a row, triangle, or zigzag. Most people recover from bites within hours to days. In people with allergies to bed bug saliva, reactions can include hives, larger swollen welts, and blisters. Scratching blisters open can lead to secondary skin infections. On rare occasions, bed bug bites can trigger anaphylaxis, a serious whole-body allergic reaction.

Cockroaches don’t typically bite humans, but they pose a different kind of health risk through their skin. Proteins in cockroach droppings, saliva, and shed body parts are potent allergens. In homes with cockroach infestations, these allergens become airborne and settle into dust, triggering asthma attacks and allergic reactions, especially in children. This is a chronic, ongoing exposure rather than an acute bite reaction.

Health Risks Compared

Bed bugs are not known to spread diseases to people. The CDC is clear on this point. Their health impact is primarily through bite reactions, sleep disruption, anxiety, and skin problems from excessive scratching. The psychological toll can be significant: insomnia and anxiety are common among people living with bed bug infestations, sometimes persisting even after the bugs are eliminated.

Cockroaches carry a broader set of health concerns. They move through sewage, garbage, and decaying matter, then walk across kitchen surfaces and food preparation areas. They can mechanically transport bacteria like Salmonella and E. coli on their bodies and in their droppings. The allergen issue is also substantial. Cockroach allergens are one of the most common indoor asthma triggers, and studies have consistently linked cockroach exposure to increased asthma severity in urban housing.

How Treatment Differs

The treatment approach for these two pests is almost entirely different, which is why correct identification matters so much before you spend money on control methods.

Bed Bug Treatment

Bed bugs are notoriously difficult to eliminate because they hide in places where traditional sprays can’t reach and have developed resistance to many common insecticides. Heat is the most effective weapon against them. Bed bugs die after 90 minutes of constant exposure to 113°F, or within 20 minutes at 118°F. Eggs are harder to kill, requiring 118°F sustained for 90 minutes.

Professional treatments typically combine multiple approaches. Whole-room heat treatments raise the air temperature in a sealed room to about 135°F using electric or propane heaters, then maintain lethal temperatures at every point in the room for at least 60 minutes. Steam treatments target specific surfaces like mattresses, upholstered furniture, carpet edges, and baseboards, with technicians moving slowly (about 12 inches every 30 seconds) to maintain surface temperatures of 160 to 180°F.

You can handle smaller items yourself. A household clothes dryer set on high will kill all bed bug life stages, including eggs, in 30 minutes when loosely filled. Portable heating devices designed for bed bugs can treat luggage, shoes, books, and other items that can’t go in a dryer by heating them above 120°F in an enclosed bag or chamber.

Cockroach Treatment

Cockroach control relies on a different strategy entirely. Because cockroaches need food and water, baiting systems are highly effective. Gel baits placed in cracks and crevices near harborage areas exploit cockroaches’ feeding and social behavior. When one cockroach eats the bait and returns to the colony, the poison spreads through contact and through other roaches feeding on its droppings or carcass. Boric acid dust applied in wall voids, behind appliances, and under sinks is another long-standing and effective treatment. Eliminating water sources (fixing leaky pipes, drying sinks at night) and sealing entry points are critical steps that make chemical treatments more effective.

Unlike bed bugs, cockroach infestations respond well to a combination of sanitation, exclusion, and targeted baiting without needing the kind of intensive heat treatments or whole-home preparation that bed bugs demand.

Quick Identification Checklist

  • Location found: Near the bed or couch suggests bed bugs. In the kitchen or bathroom suggests cockroaches.
  • Antennae: Short and barely visible points to bed bugs. Long and constantly moving points to cockroach nymphs.
  • Droppings: Smooth, ink-like black dots clustered near sleeping areas indicate bed bugs. Gritty, pepper-like specks in kitchen areas indicate cockroaches.
  • Bites: Rows or clusters of itchy welts appearing overnight strongly suggest bed bugs. No bites but worsening allergies or asthma may indicate cockroaches.
  • Speed: Bed bugs are slow crawlers. Cockroaches are fast and will sprint when disturbed.
  • Wings: Bed bugs never have wings. Adult cockroaches have visible wings lying flat against their backs.
